By completing and submitting this form, you hereby unconditionally and irrevocably:

i.    Confirm that you have informed and obtained the consent in any offline or online means from the above persons and/or companies (the “Referee”, collectively the “Referees”) in this form to allow you to disclose their name, personal and/or company email addresses and the telephone number (as set out in this form) to DBS Bank (Hong Kong) Limited (the “Bank”);
ii.    Confirm that you have informed the Referees that the purpose of such disclosure of Referees’ personal and/or company email addresses (as set out in this form) to the Bank so that the Bank to contact the Referees regarding this DBS Online Account Opening for Business “Refer-a-friend” promotion (the “Promotion”) only;
iii.    Confirm with the Referees that they have no objection to the Bank contacting the Referees for the Promotion purpose set out above;
iv.    Confirm that you have obtained the Referees’ consent for the Bank to contact the Referees via email and telephone;
v.    Consent and authorize the Bank, if the Bank so chooses, to disclose your name, company name and/or contact details to verify that you are a business account customer of the Bank to the Referees;
vi.    Confirm that you obtained, read and understood the terms and conditions set out in this form and the prevailing terms and conditions of the Promotion;
vii.    Agree to participate in the Promotion and to be bound by the terms and conditions set out in this form and the terms and conditions the Promotion.
