For selection of below plans/credit card application, please submit additional documents:

>	Photocopy of the bank statement (except online statement) or first page of passbook showing the bank name, your name and account number to which the transfer amount of the "Funds Transfer" Programme/loan amount of the "Call-a-loan" Service is transferred (joint account will not be accepted)

>	One of the following membership proof showing your name: Photocopy of the latest mobile bill statement or latest 3 months' sales and service agreement issued by 3Hong Kong

>	Photocopy of CABLE service contract proof (CABLE bundle service/i-cable homeline service/i-cable broadband service/CABLE TV service)

>	Student Card
